What a DEXA Scan Measures

A DEXA scan (dual-energy X-ray absorptiometry) passes two low-dose X-ray beams through your body to distinguish between fat tissue, lean tissue, and bone. The result is a segmented, regional breakdown of your body composition rather than an estimated single number.

Each scan produces four distinct data layers:

  • Body Fat Percentage
    Total and by region, measured directly rather than calculated from circumference or impedance estimates.
  • Lean Muscle Mass by limb
    Absolute muscle mass in each arm and each leg, independently measured. This allows tracking of true muscle gain or loss over time and surfaces left-to-right imbalances that overall weight cannot detect.
  • Visceral Adipose Tissue (VAT)
    Fat stored inside the abdominal cavity, around the organs. Expressed in square centimeters. This is the measurement most directly connected to metabolic disease risk, and the one most commonly missed by conventional assessments.
  • Android/Gynoid Ratio:
    Measure the distribution of central fat (associated with higher health risks) versus hip/thigh fat.
  • Bone Mineral Density (BMD)
    Assess bone strength and fracture risk with a precise measure of mineral content, essential for tracking long-term skeletal health.

Why body composition matters for longevity

Visceral fat predicts metabolic disease risk more reliably than weight or BMI.

Visceral fat is metabolically active, releasing inflammatory compounds and disrupting insulin signaling in ways that subcutaneous fat does not. Its accumulation is associated with elevated risk of cardiovascular disease, type 2 diabetes, and metabolic syndrome — and it is invisible without imaging. Two people at identical body weights can have dramatically different visceral fat levels and, as a result, dramatically different metabolic risk profiles.

Muscle mass is one of the strongest predictors of functional longevity.

Age-related muscle loss — sarcopenia — begins in the mid-30s and accelerates through the 50s and 60s in the absence of resistance training and adequate protein intake. Low muscle mass in midlife is associated with higher rates of disability, fall-related injury, and all-cause mortality in later life. Measuring it directly, tracking it across scans, and confirming that training is actually building or preserving it is the only way to manage it with precision.

Bone density is a decades-long project.

Peak bone density is largely established by the late 20s and declines gradually thereafter. The consequences of insufficient bone density — fractures, hospitalization, loss of independence — play out in the 60s and 70s and beyond. Building and monitoring bone density in the 30s, 40s, and 50s is the appropriate window for meaningful intervention.
