What VO₂ max measures — and what the research says about it:

VO₂ max — maximal oxygen uptake — is the maximum volume of oxygen your cardiovascular system can deliver to working muscles during sustained effort. It is the single number that most comprehensively reflects the integrated function of your lungs, heart, blood, and muscle tissue under aerobic demand.

It is expressed in milliliters of oxygen per kilogram of bodyweight per minute (mL/kg/min).

The research connecting VO₂ max to longevity is among the most consistent in exercise physiology. A 2018 study in JAMA Network Open following over 122,000 patients found that low cardiorespiratory fitness was associated with a higher risk of death than smoking, hypertension, diabetes, or coronary artery disease. A 2022 meta-analysis in the British Journal of Sports Medicine, drawing on over 20 million person-years of follow-up, found that each one-MET improvement in cardiorespiratory fitness was associated with an 11 to 17 percent reduction in all-cause mortality risk.

The association extends beyond cardiovascular outcomes. Higher VO₂ max is independently associated with reduced cognitive decline, lower cancer mortality, better insulin sensitivity, and greater functional independence in later life.

VO₂ max is also one of the few longevity markers that responds meaningfully to targeted training — which makes measuring it not just informative but actionable.

Clinical testing vs. estimated VO₂ max

Wearables and fitness apps report VO₂ max estimates built from algorithms using heart rate, pace, and population averages. These estimates can suggest a rough ballpark. They cannot produce an accurate individual measurement.

The gap between estimated and measured VO₂ max is often significant — and the direction of the error is unpredictable. An overestimate produces training zones that are too aggressive. An underestimate means easy work may not be easy enough and hard work may not be hard enough.

A clinical VO₂ max test uses a calibrated metabolic analyzer and a respiratory mask to measure the actual volume of oxygen consumed and carbon dioxide produced at each stage of a graded exercise protocol. The result is a measured, not inferred, VO₂ max — and a precisely identified ventilatory threshold.

The ventilatory threshold is what the training zones are built from.
Standard zone calculations — 220 minus age, percentage-of-max-heart-rate formulas — produce population averages. A measured ventilatory threshold produces your threshold: the specific intensity at which your metabolism shifts from primarily aerobic to primarily anaerobic. Training zones built from this threshold are accurate in a way that formula-based zones cannot be.
